Updating a previous blog post, the Department of Buildings said that there were more than three dozen violations at a Manhattan construction site where a worker fell 13 stories to his death last month. 
The accident occurred on April 2, at around 2:10 p.m., near the Dream Hotel, at 210 W. 55th St., in Midtown. The worker, Lukasz Stolarski, 34, of Brooklyn, landed on top of a sidewalk shed.
He was reportedly standing on a ledge between the roof and penthouse level when he fell, as workers were completing façade restoration work. Investigators said that he was not wearing a harness.
According to WABC-TV, multiple violations were leveled against the construction companies involved in the work, including:
- Failure to safeguard persons or property
- Failure to report an accident
- No record of daily inspection of suspended scaffold
- Work did not conform to approved plans
- Failure to provide approved plans
- Failure to provide guardrails
- Failure to provide protection
Stolarski reportedly worked for Alpha General Contractor of New York. According to WABC, there were 41 open violations at the location, including six for permits related to illegal signs. A stop work order was issued at the site by the city following the crash.
